Submitting our Mail-In form is *required* before shipping your device(s) to us. You will be emailed a "Reference Number" shortly after you have completed the form, which will put you in our system.

In order for us to get started working on your device, we must receive a submission of your Mail-In form AND your $250 deposit. You can learn more about our deposit on our Pricing page.
